1955–1961
KTHV first signed on the air on November 27, 1955; it was originally owned by the Arkansas Television Company, a consortium that included among others, the owners of radio station KWKH in Shreveport, Louisiana, the owners of the Arkansas Democrat-Gazette, and Little Rock businessman and insurance executive Clyde E. Lowry. The KTHV call letters were derived from former sister radio station KTHS (whose calls stood for "(K) Come To Hot Springs), which later became KAAY after being sold to LIN Broadcasting in the 1960s. It took the CBS affiliation from KATV, due to KTHS' long time affiliation with CBS Radio.

The font used for the KTHV call letters and the "Action News" wordmark during this period was borrowed from the logotype developed in 1973 for Phoenix NBC affiliate KTAR-TV (now KPNX), which was patterned after the proprietary logo font used by NBC News and NBC Sports from 1972 to 1976. (Robert L. Brown Sr., who held the same position at KTHV at the time, previously served as general manager at KTAR and KARK-TV, which also used the logotype before KTHV implemented it.)

KTHV remained a locally owned entity until December 1994, when the station was acquired by the Gannett Company in an all-stock transaction. Gannett's purchase of KTHV marked a re-entry into the Little Rock–Pine Bluff, Arkansas television market for the company, which previously had owned NBC affiliate KARK-TV (channel 4) from when Gannett merged with Combined Communications Corporation in 1978 until it was sold to Southwest Media in 1983. The purchase of KTHV was believed to be positioning on Gannett's part to have a station in the former home of then first-term President Bill Clinton.
1995–2013
In November 1995, KTHV adopted the "Today's THV" brand, loosely inspired by the “Today's TMJ4” moniker used by Milwaukee NBC affiliate WTMJ-TV (then the flagship station of Journal Broadcast Group, founding owner of namesake newspaper The Milwaukee Journal) from 1992 to 2020.
Despite featuring a stylized "11" emblem (derived from Los Angeles CBS O&O KCBS-TV’s 1994–97 numeral mark) in the accompanying logo, KTHV largely omitted verbal references to its VHF channel 11 allotment during this period (outside of sporadic use within its news branding such as the short-lived NewsChannel 11 title used until the “THV” moniker was extended to its newscasts in late spring 1996).
2013–present
On February 28, 2013, coinciding with its implementation of Gannett’s then-new standardized station graphics and accompanied by a new logo, KTHV revised its branding as “THV11”, removing the "Today's" participle from the name.
